After you are admitted to a law school or graduate program, you may receive a request from the school for a Degree Certification or a “Dean’s Letter”/ “Dean’s Certification” letter to be filled out. You will work with one of two offices on campus to get these documents:
For requests regarding degree completion:
Students who need certification that they have graduated from the University of Wisconsin-Madison can obtain assistance from the Office of the Registrar. This unit can assist students in filling out the Dean’s Form required by US law schools, medical schools, and graduate programs during the application process. Please make sure you provide your full name and UW-Madison student ID number when communicating with this unit.
The Office of the Registrar can provide certification/verification letters for:
- Academic standing
- Degree verification/completion letters once degree information has been posted to a student’s official records
- Enrollment verification
- Forms requiring a school/college Dean’s signature
- Military verification forms requiring a Dean’s signature

For requests regarding student conduct records:
Information about disciplinary clearance (often called a “Dean’s Letter” or ‘Dean’s Certification”) will need to be requested from the Office of Student Assistance and Support (OSAS). Students will need to make the request themselves due to FERPA that protects their records.

- Office of Student Conduct and Community Standards Certification and Recommendation Forms
